Transition Care recently helped organise a superb 21st birthday party for our service user Tom.

Tom’s support worker Dave organised the food and drinks for the party, and around 15 family and friends joined with Tom to help celebrate his special day.

A dediated football fan, Tom’s favourite present was his new Manchester United dressing gown.
